Anyone can refer a person to RSC and self-referrals are welcome.
The easiest way to refer yourself or another person to RSC is to call the closest RSC field office during regular business hours. Our staff will ask you to provide information about the individual being referred, including their name, social security number, date of birth, address, and phone number, as well as information about their disability and how it interferes with working.
If it is more convenient for you, you may complete the referral form by clicking the link below and hand deliver, fax, or mail it to the closest RSC field office.
